Please allow a minimum of 48 hrs for your submission to be reviewed and processed.

In submitting this form you are certifying that you are the parent or legal guardian of the player named below.

You understand that completion of this form, and any subsequent approval of this request, simply represents Hockey Calgary's permission to attend a Spring Training Camp or a Fall Try-out Camp and that the permission granted to participate in either Camp does not constitute a formal release from Hockey Calgary (Region 9).

Should you not be in possession of the required player Hockey ID number, please contact your Association Registrar (not Hockey Calgary) to receive the number before proceeding. If you do not know your Association Registrar a list of Association websites is available by clicking here. The number will not be auto populated or provided by Hockey Calgary.

IMPORTANT: You are required to forward via e-mail or present an actual hard copy of the Approved Letter of Permission you may receive from Hockey Calgary, directly to the Camp/Team contact person whom you have been dealing with.

I am the parent/guardian of the player named above and I approve of this Letter of Permission for the player to participate in a training or try-out Camp.

I understand this is not an actual release for the player from our primary Hockey Association, the player named above has only been granted permission to participate in a training or try-out Camp for the junior team referenced above for the period of time specified.

I agree I will send the confirmation email received directly to the team named above and/or carry it with me to the Camp to be presented to the team.

Should I be asked by the junior team above to stay on longer than the dates requested on this form I agree to contact the President of my home/ primary Association to let him/her know that the player named above still participating in the Camp.

Should this camp conflict with any of my home/primary association try-outs, it is my responsibility to adhere to the try-out policies and guidelines provided by my home/primary association.
